Rockingham Visiting Nurse Association & Hospice is a community-based, non-profit Home Health agency that serves the adult population of Rockingham County as well the communities of Barrington, Durham and Lee, NH. As an Exeter Health Resource, our agency is able to provide you and your family with the highest quality home care, hospice, and community outreach programs and services.


Rockingham Visiting Nurse Association & Hospice, an affiliate of Exeter Health Resources, was formed when the Derry Visiting Nurse Association merged with the Exeter Visiting Nurse Association in 1990.

Starting out as the Derry VNA, our agency has been caring for individuals and their families since 1905. During this period of time, small, community-based non-profit visiting nurse agencies were developed to care for neighbors in need of assistance.

We have now grown to an agency with over 130 employees, yet remain true to our long-standing history of responding to the needs of our community members. Our Care Teams provide an array of home care, hospice, and community outreach programs throughout the communities we serve.

Our Centralized Intake Department is open Monday through Friday from 8 a.m. to 5 p.m. and 8 a.m. to 4 p.m. on weekends to answer questions that you may have about home health services for you or a loved one. We also provide 24-hour on-call nursing coverage to handle necessary patient and referral calls.

The mission of Rockingham VNA & Hospice, a Home Health agency, is to assist in improving the health of the community by promoting well being and independence by providing patient-centered health care services in the comfort of the home and within the communities we serve.

One of the best solutions for care is the kind that happens in the home. If that is your situation, rely on our home care services.

When home care services are requested, a physician’s order is required to initiate the process. A clinician will perform an initial in-home assessment. Based on this assessment, we will work with you, your family, and your medical team to develop a treatment plan that will meet your specific needs.

Smoothing the transition from inpatient to home care, Rockingham VNA & Hospice team members meet with patients, family members, and health care providers at area hospitals and health care facilities to review the care patients will need, educate them about home health/hospice services, and facilitate their transition to home.

Falls Risk-Reduction Program: The Falls Risk-Reduction Program has been developed to assess and reduce the incidence of falls in elderly, at-risk populations. Patients are often referred for a safety evaluation visit by physicians, first responders, or insurance companies. Referrals may also be made by family members, friends, or other health professionals.

Complimentary educational programming is also available to community groups, providers, first responders, and other health facility personnel on falls risk-screening and reduction.


The Pre-operative Rehabilitation Services Program has been specifically designed for patients who are scheduled for elective hip or knee replacement surgery.

The program includes:
  • A comprehensive Pre-operative Evaluation performed by a Physical Therapist
  • A Home Safety Assessment
  • Teaching and recommendations for assistive devices and adaptive equipment as necessary
  • A discussion of the surgical procedure and a review of possible post-operative tracks (Home Care, Outpatient, Sub-acute Care)
  • Instruction in an appropriate therapeutic exercise regime
  • Gait and transfer training


Carefully planned interventions, patient and family education, and measuring outcomes are all components of this process. The result is often improved outcomes that include a greater ability for an individual to manage a condition; an increased compliance with treatment; and a better quality of life.

Rockingham VNA & Hospice offers specific Disease Management Programs for individuals with the following conditions:
  • Cardiopulmonary Disease, including Congestive Heart Failure (CHF)
  • Diabetes Mellitus
  • Geriatric Dementia-related disorders
  • Wounds


Palliative care involves taking care of people living with a terminal illness. It focuses on treating the whole person, not the illness, using a multidisciplinary team approach to manage the pain, symptoms, and stress that may accompany serious illness. Care may include nutritional counseling, nursing, home health aides, pharmacists, spiritual care providers, physicians, social workers, physical therapists, and volunteers.


TheAdvanced IllnessProgram emphasizes palliative care to address the many ways that serious illness affects patients and their families. The Hospice Team will put together a comprehensive, medically-directed interdisciplinary team of professionals to care for patients with progressive disease and symptom control needs and to support the families of those patients.


Care is provided in the patient’s home, using palliative care principles to treat the patient as a whole, while offering necessary support to family members and caregivers.

Health Education Programs provided by clinicians, educators, and counselors include: Adult (General) Bereavement Support Group

Provides an opportunity for those who are grieving the death of a loved one to meet in a safe, supportive environment and share with others who are also experiencing loss. Motherless Daughter Support Group
This support group allows women who have experienced this special type of loss to meet, share their emotions, and gain support from other group members.

Bereavement Services also include the following:

  • "Celebration of Life" Annual Memorial Service - Family members and friends are invited to attend this special remembrance held every spring to celebrate the lives of loved ones lost over the past year.
  • Bereavement Training
  • Bereavement Volunteers
  • Grief educational materials
  • Community Resource information
  • Workshops such as "Grief in the Workplace"

Ostomy Support Group A support group for individuals with any type of ostomy is also available, and is co-facilitated by Sandie Aguinaga, RN, CWOCN.
